4. A registration form cam be finished when registering on-site.
5. A first - time visitor needs to bring an ID card/passport, the national health insurance card, and the registration form.

On-site Registration Rules:

1. Queuing Number Cards are started to release from 07:30 A.M. and 01:30 P.M.
2. Please wait in line according to your queuing number. If you can not arrive at the counter when your number comes on, you will be treated as giving up the registration.

No Need to Report for Those who have Make an Appointment

1. Patients who have make an appointment can visit the examination room directly and enter the examination room according to the queuing number. Please show the following items：
   (1) National Health Insurance IC Card (Children's Health Manual, or Maternal Health Booklet)
   (2) Major Illness/Injury Certificate or Disability Card (if have)
2. Please a patient who takes self-bearing medical fees or forgets to carry the National Health Insurance IC Card report to registration counter first, and then enter the examination room according to the queuing number.

1. VGHTC Chiayi Branch accepts calls for making an appointment within 5 weeks and on the day which you want to enter the examination room.
2. A first-time patient can not make an appointment by phone. If you have any concern about appointment or registration, please contact the Customer Service Center (Tel: 05-2359630 ext1304, 1305).
3. If you can not visit VGHTC Chiayi Branch for your appointment, please notice VGHTC Chiayi Branch to cancel or change your appointment by phone early.
4. For preventing long waiting time, please report to the registration counter according to our personnel's suggested time.
5. All modifications of clinic must be subjected to the announcement of VGHTC Chiayi Branch.

Stage Action Prepared Data/Certificate Note 1 A physician makes out a hospitalization notification.   1.The hospitalization notification should be made out by clinic or emergency physician. 2.Clinic and emergency personnel should inquire Ward about the status of beds. 2 After receiving hospitalization notification, please report to the Registration Counter at the same day. A patient who make a appointment for hospitalization should report to the Registration Counter at the appointment date. Please prepare the following item first: 1. Hospitalization notification. Hospitalization Information

1. Before practicing hospitalization process, to keep your rights and privacy, please explicitly notice us which ward you wants, and fill the Inpatient Right Statement.
2. Please report to the Ward Nursing Station soon after completing hospitalization process. VGHTC Chiayi Branch reserves the right to cancel your hospitalization if you do not report to the Ward Nursing Station over 4 hours.
3. If you want to change your ward after hospitalizing, please submit your application to the Ward Nursing Station.
4. A patient who suffers major illness and injury and matches the condition in accordance with National Health Insurance can apply the exemption of self-bearing expenses relied on the certificate.
5. If a patient hospitalizes suffered occupational injury/disease, to keep your rights, please apply to your group insurance applicant or the Bureau of Labor Insurance for Labor Insurance Application Letter for Occupational Disease during hospitalization period. After filling the Injury/disease on the application letter by attending physician, please submit to Ward Nursing Station with your ID card. A patient who suffer traffic accident on the way for commute or business trip needs to prepare the accident description, certificate issued by the police office, and so on. Please those patients who suffer occupational injury/disease submit related data and certifications within 3 days to keep your rights.

Infection Control Policy - Regulations for Accompanying Family and Visitors
(1) Normal (non-epidemic period)

1. According to health policy, please family and visitors who have respiratory tract infection, chickenpox, or scabies do not go to VGHTC Chiayi Branch. If necessary, please wear a mask before entering a ward.
2. Please wash your hands before and after entering a ward.
3. To prevent infection, a child who is younger than 6 years old do not enter a ward.
4. To prevent cross-infection, please keep the same accompanying family who will accompany the inpatient as possible as you can.
5. Strictly prohibit any pets entering a ward.

(2) Epidemic period of infectious disease or emerging infectious disease

1. During specific epidemic period, visitors need to comply with the disease control policy of VGHTC Chiayi Branch and Competent Health Authority, such as taking body temperatures and wearing masks before entering a ward.
2. Accompanying Principle: Only 1 accompany family is allowed for any inpatient. The accompany family must register the personnel data in Ward Nursing Station, and wear the tag provided by the station. Other accompany family who do not wear the tag, or who stay in the ward in non-visiting hours, will be asked to leave the ward.
3. The accompanying family or personal carer must measure body temperature every day. In case of fever, they should see a doctor soon.
4. To prevent inpatients' rest, please visitors leave wards before 9 P.M.
5. The number of accompany family is limited to 1. Please prepare comforter and pillow. In addition, accompany family can ask the Ward Nursing Station for renting comforters and pillows for their requirement.

A nurse teaches the methods of taking medicines and attentions for discharge. If you need a medical certificate, discharge summary, or other related data, please notice the nurse or the attending physician before the date of your discharge as possible as you can. 3 Admission Counter (1F) Pay your expenses. Please prepare your medical expense pre-charge receipt. Take your National Health Insurance IC Card 4 Pharmacy (1F) Go to the Pharmacy for your medications   5 Finish and go home   If you have any concern about discharge, please contact the Discharge Counter (Tel: 05-2359630 ext1109).： 05-2359630#1109

 
Discharge Description

1. According to the National Health Insurance Act, a patient should practice discharge if the state of your illness is stable, or if you has recovery, diagnosed by the physician. Otherwise, all expenses of excessive hospitalization must be paid by the patient. Please follow the notice and practice related process for your rights.
2. After receiving your Discharge Notice from the ward secretariat, please go to the Admission Counter (1F) for paying your expenses. Then, please go to the Pharmacy for your medications, and the pharmacist will describe the methods of taking medicines and attentions.
3. If you need discharge before recovery for any reason, please notice nurses and physicians, fill the "Against Advise Discharge," practice discharge process, and then you can leave VGHTC Chiayi Branch. Otherwise, please do not leave VGHTC Chiayi Branch anyway.
4. To save your time for discharge, If you need a medical certificate, discharge summary, or other related data, please notice the nurse or the attending physician before the date of your discharge as possible as you can.
5. If you have any concern about discharge process, please contact the secretariat in each Ward Nursing Station.
